CNDC-003 IRS Form 4506-C OCR Update | Florida Capital Bank Mortgage

FLCBank will follow the Internal Revenue Service Bulletin dated 11/1/2022 which announced the final published version of the new IRS Form 4506-C. The new Form was updated to support the OCR modernization for the IVES program and will be required for IVES submissions on or after March 1, 2023. The new form cannot be edited. Requesting transcripts utilizing the new form requires the Lender to fill out the form as they intend it to be processed.

Lines 1 through 8 must be complete before the taxpayer signs and cannot be handwritten. Response lines must be submitted in Arial size 12 font.

Line 5ai, should only contain one company name and cannot include the use of Doing Business As (DBA), Care Of (c/o), Its Successors and/or Assigns (ISAOA), etc. If provided the request will be rejected.

However, as an exception, the following lines should be left blank:
